OFFICIAL LANGUAGE
The official language of the Meeting is English. All the sessions will be translated
in Italian.
EXHIBITION
The technical and pharmaceutical exhibitions will be located at the Congress Venue,
close to the Conference rooms. The exhibition hours will follow the timetable of the
scientific sessions.
INFORMATION DESK
An information desk will be available at the Congress Venue for restaurant reservations,
travel services and general information about Taormina.

The delegate registration fee includes:
Attendance at all scientific sessions including luncheon panels (limited to seat
availability), entrance to the exhibition area, meeting proceedings, attendance
certificate, CME certificate, badge and congress kit, welcome cocktail, and coffee
breaks at the Venue.

The CHF-CRT Live Implantation registration fee includes:
Participation at one day theorical/practical course with live practice of biventricular
pacemaker implantation (comprehensive of different leads for coronary sinus pacing
on manikin that resembles the venous system of the heart provided with the highest
technology and software simulating a real implantation of pacing leads), utilized
materials, surgery kit, technical and scientific assistance by an individual tutor,
entrance to the exhibition area, light lunch, attendance certificate, CME certificate,
badge and congress kit. Maximum 45 participants (April 7: 12 participants; April 8:
15 participants; April 9: 18 participants) assigned on first come first served basis,
after the full payment of the charge with date and timetables to be respected.


ECHO-CRT Course registration fee includes:
Participation at one day theorical/practical course with practical training based on
real clinical cases. The course foresees individual stations to performe echocolordoppler
and TDI practice in a model simulating a patient with heart failure and
CRT. It also includes: personal handbook of the course, utilized materials, technical
and scientific assistance by an individual tutor, free practice on a last generation
cardiac echographer, entrance to the exhibition area, light lunch, attendance certificate,
CME certificate, badge and congress kit. Maximum 48 participants (16 persons per
day) assigned on first come first served basis, after the full payment of the charge
with date and timetables to be respected.


The BLS-D Course registration fee includes:

Participation to the Course (from 9.00 to 18.30), entrance to the exhibition area,
BLS-D licence, CME certificate, coffee breaks and light lunch taken at the Venue.
Maximum 20 participants assigned on first come first served basis, after the full
payment of the charge.


The accompanying person registration fee includes:
Entrance to the exhibition area, welcome cocktail and half day excursion.

To register (also for BLS-D, CHF live implantation and ECHO-CRT courses) it is
necessary to fill in the registration form, send it to Adria Congrex with the full
payment. Post mark certifies mailing date.


DELEGATES FAILING TO REGISTER BY MARCH 20 2005 MAY BE ABLE TO
REGISTER AT THE CONGRESS VENUE.

HOTEL ACCOMMODATION
The hotel accommodation form must be filled in and sent to Adria Congrex with the
hotel deposit. As Taormina is a world-wide famous tourist resort, and April is the
opening holiday season, it is highly recommended to book well in advance, in any
case no later than January 15, 2005. Adria Congrex will make every effort to place
you in the hotel category of your choice. Hotels in Taormina have very few single
rooms at disposal, therefore, in case single rooms are no longer available, double
rooms will be assigned as single. Once hotel availability in Taormina is over, they
will be assigned in Taormina Mare. A confirmation will be sent starting from
February 15, 2005.
Please note that hotel categories and rooms are assigned on a first come first served
basis.
A deposit for the first night is required to process your reservation.
The deposit paid will be deducted from the hotel bill, payment in full of the balance
is due at check out. The hotel will provide official receipt for the whole amount.

All prices include: taxes, service, continental breakfast and are per room per night.


PAYMENT CONDITIONS
Accepted payments are: cheque written on Italian Banks, credit cards, Bank transfer
in Euro (made out to Adria Congrex Banca Carim ag. 14 IBAN code IT81B0628524214
001402118559 – swift code CRRN IT 2R). All costs to transmitter's charge.
Please indicate clearly and in full your name, address and MCM2005 on cheques
and money orders.
Registration and hotel reservation will be confirmed only on receipt of the amounts
due. Adria Congrex will provide regular invoice for registration and reservation fees.
The form should therefore be fully completed with the European VAT code (only for
EU countries) and the invoice address.

CANCELLATION POLICY
Any cancellation should be notified in writing to Adria Congrex and will be subject
to the following conditions: Before December 30, 2004.
Refund of registration is subject to a 30% administrative charge, while for hotel
deposit no refund will be made. All refunds, if any, will be made after the congress
(all costs to receiver's charge).
After December 30, 2004: no refund of any kind.
No refund in any case will be made for BLS-D, ECHO-CRT and CHF Live Implantation
Courses registration.


CHANGES in PROGRAM
For scientific and/or technical reasons the Scientific Committee and the Organising
Secretariat reserve the right to make any change in the Meeting program.
